藝術校 2025親子旅遊報名表 NYAS 2025 Field Trip

一年一次的親子旅遊，終於開跑囉！Let's Go!

我們將帶著寶貝一起探索『五峰旗瀑布』+『潭酵天地觀光工場』

活動注意事項：

1. 參與人數：每位在校生『最多』2位大人、2位小朋友(1位在校生+1位2至6歲兄弟姐妹)參加。

2. 如有兄弟姊妹之家庭，請以『兄姐』班級，填寫報名

3. 報名時間：5/14 10:00 ~ 5/16 18:00 報名完成以後，將確認資料，如有報名成功，將於5/20 收到APP繳費通知。

4. 報名費用：

✔︎ 國小至大人 1200元
✔︎ 2歲至6歲小孩（含在校生）900元
✔︎ 不佔座位 0-2歲 嬰兒 人數   200元（保險費）

5. 繳費期限：請於 5/20-5/22 以前完成繳費，並請上傳繳費證明。未於期限內繳費，將放棄名額。

若已繳費，但於5/30 以前 臨時變異，不退費。

6. 候補報名： 5/23 10:00 am~5/26 18:00 pm.

7. 候補繳費日期為：5/27~5/29

活動注意事項：

1.當日請寶貝穿著『學校運動服』，以維護寶貝出遊安全！

2.因活動有投保個人意外險，報名填寫資料送出前務必確認【姓名】、【身分證字號】、【出生年月日（西元）】，請家長務必填寫清楚以上資料。

3.請留意天候變化，自行攜帶隨身物品、藥品、防曬、防蚊蟲用品及雨具。

4.本活動如遇颱風、地震、豪雨等不可抗力之天災，將以新竹市政府發布停班(課)之規定辦理；即活動當日如發布停班(課)，則停止相關活動，異動後之活動日期由學校另行通知。

6.本活動若有未盡之事項，主辦單位保留活動辦法之修改、變更之權利。

We are excited to take you and your little ones on an adventure to:

"Wufengqi Waterfall" + "Tang Jiao Tian Di Tourist Factory"!

📌 Important Event Details:

1. Participants Limit:
   Each enrolled student may register a maximum of 2 adults and 2 children (1 current student + 1 sibling aged 2-6).

2. Please register families with multiple siblings in our school under the Elder sibling's class.

3. Registration Period:
   May 14, 10:00 AM – May 16, 6:00 PM
   Once registered, we will review the information. Successful registrants will receive a payment notice via APP on May 20.

4. Fees:

   • Elementary students to adults: NT$1200

   • Children aged 2–6 (including enrolled students): NT$900

   • Infants aged 0–2 not occupying a seat: NT$200 (insurance fee only)

5. Payment Deadline:
   Please complete payment and upload your payment receipt by May 22.
   Failure to pay on time will result in the cancellation of your spot.
   If you cancel after May 30, no refunds will be given.

6. Waitlist Registration:
   May 23, 10:00 AM – May 26, 6:00 PM

7. Waitlist Payment Period:
   May 27 – May 29

📌 Reminders for the Event:

1. All students should wear their school sports uniform to ensure safety during the trip.

2. Since personal accident insurance is included, please double-check the following information before submitting your registration:

   • Name

   • National ID number

   • Date of Birth (Gregorian calendar)
     Please ensure all information is accurate.

3. Prepare for weather changes and bring personal items such as medication, sun protection, insect repellent, and rain gear.

4. In case of typhoon, earthquake, heavy rain, or other force majeure events, the activity will follow the Hsinchu City Government’s announcement on work/class suspension.
   If such announcements are made on the event day, the trip will be canceled. The school will announce any rescheduled date.

5. Lunch will be self-catered.

6. The organizer reserves the right to modify or adjust the event details.
