國立嘉義大學112-1學期「嘉義巡禮」通識微學分課程(校外)報名表

對象：限國立嘉義大學大學部學生。未額滿課程於課程開始前7日亦開放本校研究所學生與教職員參加。
Participants: Only for undergraduate students at National Chiayi University. Unfilled courses will be open to graduate students and staff till 7 days prior to course start.

報名期程：9月13日（星期三）13：00起至額滿為止，課程若尚有名額，將開放至該課程開始前3日。
Time: Start from September 13 (Wednesday) 13:00. It will continue open registration till 3 days prior to course start.

注意事項：如無法參加課程，務必於前3日通知教務處通識教育中心(discoveringchiayi@gmail.com)及授課教師，無故缺課將列為黑名單，禁止報名下一學期嘉義巡禮課程。

Note: If you are unable to attend the course, you must notify the General Education Center of the Academic Affairs Office(discoveringchiayi@gmail.com) and teacher 3 days before the course start. Unexcused absences will be listed on the blacklist, and registration for the Discovering Chiayi course in the next semester will be prohibited.

提醒：非本國生-「身分證字號」欄位請填入「居留證號碼」，並務必上傳居留證照片，俾憑辦理保險。
If you are an international student - please fill in the number of "R. O. C. (Taiwan) Resident Certificate " in the " ID No." column, and please upload a photo of your residence permit, we will use it for insurance purposes.
