Bradley D. Fullerton, MD |
Dr. Fullerton obtained his MD from TheUniversity of Texas Southwestern Medical School in Dallas and PM&Rresidency at Graduate Hospital/University of Pennsylvania in Philadelphia. Hehas been in a primarily musculoskeletal private practice in Austin, TX since1997 while pursuing varied interests such as starting Austin’s firstneuromuscular pulmonary clinic, serving 12 years as medical director for thepediatric spasticity clinic at Dell Children’s Medical Center and researchopportunities/training with University of Pittsburgh and University ofWisconsin, Madison. 報名方式：
1.報名時間及名額：108/11/21 13:00 開放報名。
2.於Beclasss報名系統填寫報名資料後，學會依報名順序傳送繳費通知，請於收到通知3日內繳費以
完成報名，於最後期限內未完成繳費者，經通知仍未繳費，將取消報名資格，由學會依報名順序遞
補。
3.Beclass報名系統可查詢是否報名成功。
4.退費約定：
＊本會非因不可抗拒之天災人禍而無法開課，則該堂課之報名費全額退費。
＊因報名者個人因素無法參加者，請提早以e-mail或電話通知本會退費。若有名額可遞補則扣除手
續處理費後再退費。
＊距開課日前30日取消者酌收處理費500元；開課前21~30日前取消報名者退90%；開課前
8~20日取消報名者退70%；開課前4~7日取消報名者退50％；3日內取消或未到者除不可抗拒
之因素則概不退費。

How to register:
1.Registration time: Registration starts online on 23 November 2019, available on first come
first served basis.
2.After filling out the registration form through BeClass registration system, we will send
a payment notice. Please transfer the fee within 3 days of receiving the notice to
complete the registration. Those who fail to complete the payment within the deadline
will be disqualified from the registration.
3.You can check BeClass registration system to see whether your registration is successful.
4.Refund agreement:
★ On condition that the course fails to start due to any force-majeure circumstances,
the registration fee can be refunded in full.
★ If you are unable to participate due to the personal reasons, please inform us in
advance by e-mail (there's no making up of the course).
★ When the personal factor refund request is received, the full refund can be made if
the vacancy can be filled, according to the order of registration.
★The cancellation fee is USD$70. Cancellation 21 to 30 days before the start of the
course will be refunded 90% (cancellation fee required); cancellation 4 to 20 days
before the start of the course will be refunded 70% (cancellation fee required);
cancellation within 3 days of the start of the course will be refunded 50%
(cancellation fee required); those who failed to arrive will not be refunded except for
irresistible factors. Renewal fee; those who have not arrived will not be refunded
except for irresistible factors.
