103-2 Semester-Application for Student Dormitory/103-2學期-住宿申請表:

註：中學部G10~G12為規定住宿年級，需繳回住宿確認單或不住宿申請單並檢附證明『紙本』，毋需填寫以下『線上』申請表。
This application is ONLY an application, NOT a garantee of residency.
此表單僅為申請，此申請並不表示已排入床位。

1. Please reconfirm your personal information before you send out the application, especailly email, or any related information.
2. Those who failed to apply before the due date (November 30th) will be moved to the waiting list with the lower priority. Please contact the Student Dormitory Concil to make the applicaitons.
3. The application of the dormitory lasts for only one semester (according to the school calendar). Fridays , summer vacations, and winter vacations are NOT included. All the applicants are divided by the following dorm priority sequence.
1. 請確認個人資料正確性（如email），以免錯失住宿相關訊息。
2. 未於公告期限內(11/30)申請者，降一順位；逾期申請請與舍務組聯絡，列為候補名單。
3. 住宿申請以一學期為限(依學校行事曆規定)，『不含星期五及寒暑假』。舍務組受理住宿之申請後，得依以下住宿優先順序分配宿舍。

4. Dorm Priority Sequence/ 住宿優先順序：
4.1 All students (Grade 10-12) for the GSAT (General Scholastic Ability Test) of the secondary program department share the first priority.
4.2 G9 students preparing for the CAP (Comprehensive Assessment Program) of secondary department share the second priority.
4.3 Former boarders. (Assorted by the distance between their home and school)
4.4 Foreign or far-away students.
4.5 Students with private issues.
4.6 School team members.

4.1 學測班(中學部G10-G12)學生。
4.2 中學部九年級會考班。
4.3 原住宿生(若床位不足則依距校里程安排)。
4.4 眷居遠程學生。
4.5 家庭特殊因素者。
4.6 學校代表隊。

※Due to the Personal Information Protection Act, you DO NOT need to fill out your phone and address on the following form. If necessary, we contact parents via following EMAIL or I-SCHOOL information.
If you change your phone number and address, please contact with Undergraduate Academic Affairs Division and update the information in case of missing important notices!
