An Introduction to the Integrated Systems Model for the Complex Patient
Subtitle: A clinical reasoning approach to determine where to begin treatment for those with multiple impairments and persistent pain
Course Description
Understanding the relationship between, and within, body regions and systems (neural, articular, myofascial, visceral etc.) and the consequences of impaired function of one region/system on another is complex. Many health practitioners specialize in body regions, and/or systems and treatment is often based on the practitioner’s training and experience. However, this reductionistic approach may not be optimal in that each body region is not only an integrated system within itself (i.e. comprised of many differentiated yet linked parts) but is also integrated, and interdependent, as part of the whole body/person.  No studies have correlated persistent pain anywhere in the body to a consistent impairment. Therefore, in persistent pain conditions such as low back pain, metatarsalgia, TMD, headache etc., understanding what body region and system should be treated requires an individual clinical reasoning approach. The Integrated Systems Model is an evidence-informed, clinical reasoning approach that embraces, and enhances, the concepts of a regional interdependence model.  This 2-day course will introduce the participant to ISM through lectures and demonstrations and provide a structured clinical reasoning model for understanding where to focus treatment for those with multiple impairments and persistent pain. 
COURSE OBJECTIVES
At the conclusion of this course, you will understand:
1.How the Integrated Systems Model is used to assess function of the whole body/person, and thus the relationship between sites of impairment (biopsychosical considerations).
2.The concepts for finding ‘drivers’. When there are multiple regions that are impaired, how do you determine which one should be the focus of your intervention?  Is this a hip problem that is causing the pelvic joints to lose control and become painful (or alternately a thorax or foot problem), or is the impairment intrinsic to the pelvis itself (i.e. stiff SIJ or altered motor control of pelvic floor/transversus abdominis – a pelvic driven pelvis)?
3.How to further assess the body region primarily driving the condition to understand the specific underlying system (articular, neural, myofascial, visceral, physiological etc) to treat/train.
4.The concepts for designing a multimodal treatment program (including education, manual therapy, neuromuscular release, and movement training) to restore function and performance for any patient (from postpartum moms to elite level athletes) since the principles of the ISM approach are applicable to all groups of patients. 
Course Schedule
Day 1 Possible Outline 
1.Introduction to regional interdependence and the Integrated Systems Model – 2 hour Lecture
2.The Pelvis – A deeper look at the pelvis and its relationship to impairments of function elsewhere in the body – 1.5 hour lecture
3.The Thorax – A deeper look at the thorax and its relationship to impairments of function elsewhere in the body – 1.5 hour lecture
4.Live demonstration of an ISM assessment for an individual with dysfunction/pain followed by a short lecture on Principles of ISM treatment and then a short treatment of the patient – 1.5 hours
Day 2 Possible Outline 
1.The abdominal wall and its relationship to the thorax and trunk function - lecture with case presentations including the role of ultrasound imaging in both assessment and treatment of the abdominal wall. 2 hour lecture
2.Diastasis rectus abdominis and trunk function – the vital role of fascia in both assessment and treatment/training 1.5 hour lecture
3.Live demonstration of an ISM assessment for an individual with an abdominal wall dysfunction (will need to have patients provided (with consent) for both live demonstrations) 1 hour approx.
4.Principles and examples of motor learning and movement training integrated with ISM
